Expert consensus on emergency end expiratory carbon dioxide monitoring

End tidal carbon dioxide (ETCO2) monitoring is a non-invasive, simple, real-time and continuous functional monitoring index. With the miniaturization of monitoring equipment, the diversification of sampling methods and the accuracy of monitoring results, ETCO2 has been more and more widely used in the clinical work of emergency department. Its clinical application is as follows:

1.Determine intubation position

Artificial airway positioning, after endotracheal intubation, use ETCO2 monitor to judge the intubation position. Nasogastric tube positioning: after nasogastric tube intubation, use bypass ETCO2 monitor to assist pipeline positioning to judge whether it enters the airway by mistake. Monitoring ETCO2 during the transfer of patients with endotracheal intubation to help judge the ectopic of artificial airway can timely find the ectopic release of endotracheal intubation and reduce the risk of transfer.

2.Ventilation function evaluation

Low ventilation status monitoring and real-time monitoring of ETCO2 during low tidal volume ventilation can timely find carbon dioxide retention and reduce the frequency of arterial blood gas examination. Monitoring of high-risk patients with hypoventilation and ETCO2 in patients with deep sedation, analgesia or anesthesia. Airway obstruction judgment: use ETCO2 monitor to judge small airway obstruction. Optimizing ventilation conditions and continuously monitoring ETCO2 can timely find hyperventilation or insufficient ventilation and guide the optimization of ventilation conditions.

3. Evaluation of circulation function

Judge the recovery of autonomic circulation. Monitor ETCO2 during cardiopulmonary resuscitation to help judge the recovery of autonomic circulation. Judge the prognosis of resuscitation and monitor ETCO2 to help judge the prognosis of resuscitation. Judge the capacity reactivity and jointly evaluate the capacity reactivity using ETCO2.

4.Auxiliary diagnosis

Pulmonary embolism screening, ETCO2 was monitored during pulmonary embolism screening. Metabolic acidosis. Monitoring ETCO2 in patients with metabolic acidosis partially replaces blood gas analysis.

5.Condition evaluation

Monitor ETCO2 to help assess the condition. Abnormal ETCO2 values indicate critical illness.
